To answer your questions,
-Cunard does add the hotel & dining charges automaticly to your shipboard account. If you wish to tip individually, you can request that it be taken off or reduced. This does not include 15% grautity on bar and spa bills.
-Shore excursions are reserve and pay onboard. You will receive a booklet of options in your documents prior to cruising so you have time to decide what you want. Another option and one I find to usually be a better value, depending on what you are looking for, is to book through a third party such as www.shoretrips.com
Just be sure to book something with plenty of time before and after the tour to allow for any delays. (ie, if you have 7 hours in port book a 4 hour tour)
-Extra costs include: Todd English, soda & alcoholic beverages, specialty coffees, the casino and shops, spa services and shore excursions. If I have forgotten anything I will post again.
